Facilities Rental Agreement Form

By submitting this form, the User acknowledges receipt of the Safe Sanctuary Policy and agrees to abide by its guidelines, responsibilities, requirements, and restrictions. The User also agrees to provide proof of liability insurance in the amount of $300,000. In place of proof of liability insurance, the User may sign the Release and Indemnification agreement below.

The undersigned acknowledges that Knob Noster United Methodist Church does not carry medical-pay coverage for its premises and that the listed activity may result in personal injuries to participants. The undersigned further acknowledges that the activity is outside the supervision of the church and assumes the risk of injuries resulting from use of the Fellowship Hall and/or Jubilation Center.

In consideration of use of the facilities, the undersigned releases the church, its employees, members, volunteers, and agents from legal liability for damage, injury, death, or claims based upon negligence arising from or related to the listed activity.

If any person not a party to this agreement makes a claim or files a lawsuit against the church, its employees, members, volunteers, or agents relating to the listed activity, the undersigned agrees to indemnify, defend, and hold them harmless from those claims and lawsuits, including damages, expenses, costs, and attorney fees.
